- [Coldcard entropy failure, July 2026](https://proofofcustody.io/incidents/coldcard-entropy-2026): Live status page for the hardware wallet firmware defect that caused affected devices to skip their hardware random number generator and fall back to a predictable software generator. Keys generated on affected firmware can be reconstructed offline. EVERY MODEL IS AFFECTED: Coinkite's advisory, updated August 1 2026, puts Mk2 and Mk3 at roughly 40 bits of effective entropy and Mk4, Q and Mk5 at roughly 72 bits, against an intended 128; Block's engineering analysis reports the secure-element reseed contributes at most 32 bits. The two sources agree on scope and differ on magnitude. Fixed firmware: 4.2.0 (Mk2/Mk3), 5.6.0 or Edge 6.6.0X (Mk4/Mk5), 1.5.0Q or Edge 6.6.0QX (Q). Patched firmware does NOT repair a seed already generated on vulnerable firmware. Galaxy Research's August 24 2026 tally puts high-confidence losses at 1,789.28 BTC, about $114.7M at the prices when the coins moved, from 8,865 addresses, on 221 victim reports and on-chain analysis; including medium-confidence cases takes it to roughly 1,824 BTC. Roughly 90% of the stolen coins have not moved. On September 21 2026 white-hat operators swept 52.37 BTC, about 3% of the total, into an address presented as the Crypto Recovery Trust (block 967,948); no claims process, trustee or deadline has been published, so the coins are out of the attacker's reach rather than returned. Coinkite states the defect sat at a boundary between two unrelated submodules, outside the cryptographic and Bitcoin-specific logic that most internal and third-party reviews target, and that AI-assisted code review did not catch it either before or after the incident.
